BACKGROUND:

The Women in Transplantation Steering Committee has created the Woman Leader in Transplantation Award to recognize a woman who has advanced the field of transplantation through leadership, dedication, mentorship, science or other outstanding contributions.

PURPOSE/DESCRIPTION:

To provide recognition to one woman who has helped further the field of transplantation through research, policies, leadership, initiatives or other regarded contributions.
The award will be presented to one woman during the 2014 World Transplant Congress in
San Francisco, California, USA.

NOMINATION GUIDELINES:

The nomination process is as follows:

• Complete the nomination form, collect the supporting documents and retain copies for your records.
• Once reviewed, nominees and nominators will be advised by email whether the nomination has been accepted.
• There is no provision for posthumous nominations.
• Nomination packages must be received by May 30th, 2014 to be considered.

2. STATEMENT OF NOMINATION

Please respond to each of the three questions below. Please do not exceed two pages, using
12-point font to answer the questions.

To properly showcase the achievements of the nominee, we encourage you to use specific examples demonstrating the impact of the achievements. All nomination submissions are evaluated based on the submitted nomination package, which outlines the nominee’s leadership, her dedication to providing mentorship to other colleagues and for her notable achievements in the field of transplantation.

1. What are the nominee’s unique and outstanding achievements in the field of transplantation?
2. How has the nominee contributed as a mentor or role model for other women in the field? Please use specific examples.
3. How has the nominee contributed to the advancement of women in the field or as leaders in transplantation?

3. LETTERS OF SUPPORT

Please provide at least one and up to a maximum of three letters of support which further demonstrate the impact the nominee has made.

• Letters should not exceed one page in length
• Letters of support must be signed by the letter writer

4. PHOTOGRAPH

Please submit a recent photograph of the nominee in digital format (675 x 900 pixels),
preferably a headshot.

5. RESUME

Please attach a resume, which outlines the nominee’s background, career and present work including organizational involvement, volunteer activities, education, awards, recognition, publications and other achievements.

AWARD:

• $2,500 USD cash prize
• Complimentary registration to WTC 2014
• Recognition during TTS’ Presidential Address (Tuesday, July 29, 2014)

OTHER IMPORTANT REQUIREMENTS AND TIMELINES:

• The Steering Committee will inform the awardee in writing before June 16th, 2014.
• The awardee must attend the TTS Awards Ceremony at the World Transplant Congress to formally accept the award (time and date to be determined).
