The Health Education and Learning Platform (HELP) was developed to support transplant recipients in building the knowledge, skills, and confidence needed for successful long-term self-management. Created through collaboration among patients, caregivers, healthcare professionals, and researchers, HELP provides accessible, evidence-based education through a mobile application. Key features include a customizable educational library, medication and health reminders, and tools designed to support engagement and adherence.
This presentation will describe the history and development of HELP, including the rationale for creating a digital platform to address common challenges faced by transplant recipients during transitions in care and increasing self-management responsibilities. The presenters will also provide an overview of a multicenter study evaluating HELP in adolescent and young adult kidney transplant recipients. Study design, implementation, and key findings related to usability, engagement, and patient outcomes will be discussed. In addition, the session will explore practical challenges for allied health professionals and lessons learned during the research process.
