Type 1 diabetes is an autoimmune disease characterized by T-cell–mediated destruction of pancreatic beta-cells. Islet transplantation is an effective therapy, but its success is limited by islet quality and availability along with the need for immunosuppression. New approaches include the use of stem cell–derived insulin-producing cells and immunomodulatory therapies, but a limitation is the paucity of reproducible animal models in which interactions between human immune cells and insulin-producing cells can be studied without the complication of xenogeneic graft-versus-host disease (xGVHD).

Obesity is a growing issue that is spreading worldwide; its prevalence is ever increasing in patients with end-stage renal disease and represents a potential barrier to transplantation. The lack of unanimous guidelines exacerbates the current disparity in treatment, which can affect outcomes, leading to a significantly longer time on the waiting list. Multidisciplinary and multimodal management (encompassing several healthcare professionals such as nephrologists, transplant physicians and surgeons, primary care providers, and nurses) is of paramount importance for the optimal management of this patient population in a continuum from waitlisting to transplantation.

The Birt-Hogg-Dubé (BHD) syndrome is a rare, inherited, autosomal dominant disorder that was first described by Birt et al in 1977. It is characterized by cutaneous lesions on the face and neck, multiple pulmonary cysts, and renal cell carcinomas (RCCs). Furthermore, renal “oncocytosis” is a specific feature of patients with BHD syndrome; these nodules are presumed to be precursor lesions to malignant tumors. Renal lesions are the major cause of morbidity and mortality in BHD syndrome, and their diagnosis and management are crucial. Herein, we report a case of BHD syndrome with RCC development before and after living-related kidney transplantation. To our knowledge, this is the first report of BHD syndrome in a recipient of kidney transplantation.

Donor-derived transmission of infections is a rare complication of kidney transplant. Hepatitis A virus (HAV) is a common cause of acute viral hepatitis worldwide, but donor-derived transmission to organ recipients has been reported in the literature only twice previously. The timeline for HAV incubation and clearance in transplant recipients is not well understood.
