Chronic Rejection Series: Heart Cardiac Allograft Vasculopathy
Cardiac allograft vasculopathy (CAV) is a leading contributor to late allograft failure, mortality, and morbidity after heart transplantation. The prevalence of moderate to severe CAV over time has not changed significantly, and CAV remains an important contemporary consideration in post–heart transplantation management. The diagnosis and surveillance of CAV include both established and emerging invasive and noninvasive modalities, with a growing interest in understanding microvascular function and coronary physiology.
CCL22-expressing Stem Cell–derived Islet Grafts Recruit Regulatory T Cells in Mice
Cell therapy using human donor or stem cell–derived islets (SC-islets) to replace lost insulin-producing beta (β) cells holds great promise for type 1 diabetes. Recruiting regulatory T cells (Treg) through chemokine signaling could mitigate allo- and autoimmune attack on transplanted β-cells, potentially obviating the need for immunosuppressants. We hypothesized that SC-islets genetically engineered to secrete the chemokine C-C motif chemokine ligand 22 (CCL22) would attract Treg to the site of transplantation and may ultimately prolong graft survival.

Prolonged Skin Graft Survival and Histocompatibility in Highly Inbred Miniature Swine
Pigs are potential organ donors for humans. Some proposed xenotransplant tolerance regimens require genetically identical cells from different animals (eg, juvenile bone marrow and mature organs or tissues). We therefore sought to develop a highly inbred line of miniature swine for this purpose. The aim of this study was to test histocompatibility in a new subline of highly inbred miniature swine.
Optimizing Kidney Allograft Years: Underutilization of Pediatric Donors
The Kidney Donor Profile Index (KDPI) score drives kidney allocation in the United States for both adult and pediatric donors, despite the calculation being primarily derived from adult donor data. A KDPI of ≤20% is considered to have the longest graft survival potential and is typically prioritized for recipients who have a low Estimated Posttransplant Survival (EPTS) score. Using KDPI to assess pediatric donor quality may underestimate graft survival and result in underutilization of these kidneys for recipients with a low EPTS score.
