Just Released - Transplantation Direct - February Issue

The February Transplantation Direct issue covers a wide spectrum of topics in transplantation. Organ donation and procurement continues to be emphasized in our journal, where this issue has an article on supporting kidney function during long-term normothermic liver machine perfusion, and two articles on using normothermic perfusion for combined heart and liver transplantation. Another paper focuses on development of an education program in Korea to teach students about organ and tissue donation. In liver transplantation, a new procedure for using transesophageal echocardiography during transplantation is presented, and in lung transplantation a study looks at whether post-transplant lung embolism associates with chronic lung allograft dysfunction. In the kidney, we have articles that focus on long-term data in transplant recipients and on a personalized stratification approach in patients to optimize long-term outcomes. Also, an experienced transplant center reports on results using intestinal transplantation in patients with familial adenomatous polyposis syndrome. Optimization of Kidney Health in Liver Transplant Candidates: Pretransplant Considerations and Modalities
Patients with decompensated end-stage liver disease (ESLD) are at increased risk for mortality, and only liver transplantation (LT) offers meaningful hope for survival. These patients are at risk for kidney dysfunction through the continuum of care for ESLD including LT. We discuss the role of accurate estimation and measurement of baseline glomerular filtration rate in assessment of kidney dysfunction among those with ESLD.
Long-term Effects of Kidney Transplantation Compared With Dialysis on Intima–media Thickness in Children—Results From the 4C-T Study
Children requiring kidney replacement therapy experience high burden of cardiovascular (CV) disease leading to increased mortality. Intima–media thickness (IMT) indicating atherosclerosis is a validated surrogate marker for future CV events.

Canadian Kidney Transplant Professionals’ Perspectives on Precision Medicine and Molecular Matching in Kidney Allocation
Antibody-mediated rejection is an important cause of kidney transplant loss. A new strategy requiring application of precision medicine tools in transplantation considers molecular compatibility between donors and recipients and holds the promise of improved immunologic risk, preventing rejection and premature graft loss. The objective of this study was to gather Canadian transplant professionals’ perspectives on molecular compatibility in kidney transplantation.
Early Metabolic Measures Predict Long-term Insulin Independence in Recipients of Total Pancreatectomy and Islet Autotransplantation
Although diabetes after total pancreatectomy and islet autotransplantation (TP-IAT) is one of the biggest concerns for TP-IAT recipients and physicians, reliable prediction of post-TP-IAT glycemic control remains unestablished. This study was conducted to identify early predictors of insulin independence and goal glycemic control by hemoglobin A1c (HbA1c) ≤ 6.5% after TP-IAT.
